Job Summary

The district health Information Officer shall mentor district medical clerks in proper capture and management of HMIS data collected from health facilities and communities. They will also provide support in entry, cleaning and analysis of data captured for project purposes.

A. Support and mentor the health facility medical records clerks in the following:

1. Recording patient visits and other registration data in appropriate HMIS tools
2. Preparing patient files for the clinic day
3. Filing all patient files at the end of the clinic
4. Filing all laboratory test results and other records in their appropriate files
5. Ensuring that the medical records room is orderly and all files accounted for
6. Ensuring completeness, consistency and accuracy of data captured in the HMIS
7. Electronic capture of patient care records
8. Timely compilation of the routine HMIS reports
9. Ensuring availability of required HMIS tools i.e. registers, forms, guidelines etc

B. Provide assistance in the following tasks:
1. Perform data abstraction from the HMIS and other sources for project reporting
2. Perform routine data entry into the project electronic management information systems
3. Ensure completeness, consistency and accuracy of data captured in the project MIS.
4. Participate in analysis and interpretation of data
5. Perform routine backups and ensure security of data captured into the project HMIS

C. Any other assigned duties in line with your role
