We currently have an exciting career opportunity for the position of Senior Care Manager, Medical Department.

Role Purpose
The job holder will be accountable for Care Management of our Clients through the management of Client’s Health Informatics admissions incorporating with wellness, chronic disease management and outpatient patient reviews.

1. To innovate ways of reducing re-admissions , frequency of admissions and length of stay
2. To provide guidance in discharge policy and enrollment of clients post-admission into various programs
3. Provide a road map in offering value-based care as opposed to fee-for service in Care Management
4. To ensure our clients receive quality care by matching their needs with appropriate services
5. To co-ordinate Care Managers in scheduling and assign work plans
6. To ensure efficiency in work flow of back office pre-authorization and services based on benefits and exclusions
7. To supervise overseeing of patients at clinics, hospitals and other facilities through the Field Care Managers
8. Leading Care Management meetings
9. To monitor re-admissions, length of stay and hospital acquired infections
10. To supervise discharge policy and enrollment of clients post-admission into various JIC programs
11. Advising management on any peculiarities noted in the care management noted from the group providers
12. Client presentations and member education on wise utilization & risk management
13. Managing the 24-hour emergency helpline
14. Providing updates to all providers countrywide on membership data and scheme rules and exclusions
15. Provider audits on regular basis to ensure acceptable standards and practices for Jubilee clients
16. Assisting Regional offices in coordinating care for regional clients in Kenya
17. Providing technical support to the regions- assisting on non- routine claim queries and streamlining processes
