- Contribute to the deinstitutionalisation of children and ensuring they are placed in family-based care.
Support the team in identifying and planning interventions in institutions and families for deinstitutionalisation (placement, prevention of family breakdown or child abandonment and post-placement follow up).
Support the team in identifying and planning and delivering interventions for deinstitutionalisation.
Contribute to child and family assessment, case management and placement decision, preparation of children and families, and post-placement support
Provide advice and guidance to support the development of the deinstitutionalisation model
Together with the team, plan and document work plans and exit strategies in conjunction with the overall strategic aims and priorities of the organisation
Together with the team, extract and document learning for the purposes of sharing this with other focused organisations and service providers.
To deliver good outcomes for disabled children through taking referrals, completing timely assessments and implementing effective service plans that safeguard vulnerable children.
To assess and promote the life chances of disabled children, including children in care their health, education and leisure needs.
- Undertake any other activity that contributes to the smooth running and development of the organisation.
