Qualifications

· Bachelor of Pharmacy (B.Pharm) from a recognized institution.

· Active pharmacy license in the applicable jurisdiction

· Postgraduate certificate or residency in clinical pharmacy or pharmacotherapy (highly desirable).

· Minimum of 3 years of post-registration experience in a hospital setting, with significant exposure to clinical pharmacy services.

· Proven knowledge of clinical pharmacology, therapeutics, and evidence-based medicine.

· Proficiency with pharmacy information systems and Microsoft Office applications.

Responsibilities

Clinical Pharmacy Services

· Conduct comprehensive medication reviews and reconciliation for patients admitted to General Medicine and Surgical wards.

· Provide evidence-based recommendations on drug therapy, dosage adjustments, and monitoring to physicians, nurses, and other healthcare professionals.

· Monitor patient responses to therapy, identify and resolve medication-related problems, and document clinical interventions. Participate in ward rounds, multidisciplinary team meetings, and patient care conferences.


Patient Safety and Quality Assurance

· Champion antimicrobial stewardship programs to promote appropriate antibiotic use and combat resistance.

· Participate in medication safety initiatives, including adverse drug event reporting and analysis.

· Ensure compliance with hospital formularies, treatment protocols, and national/international clinical guidelines. Uphold the highest standards of patient confidentiality and ethical practice.



Education and Counselling


· Provide clear and concise medication counselling to patients and their caregivers to ensure understanding and adherence.

· Educate doctors, nurses, and junior pharmacy staff on new medications, protocols, and best practices in pharmacotherapy.

· Contribute to the development of educational materials and guidelines for the pharmacy department and wider clinical teams.

Operational and Dispensing Support

· Supervise and verify the accurate dispensing of medications for inpatients, ensuring correctness of drug, dose, and direction.

· Manage and advise on the use of high-risk medications (e.g., anticoagulants, opioids, chemotherapeutic agents).

· Collaborate with the procurement team to ensure availability of essential medicines and address drug shortages.
