Work Schedule:


Monday to Friday, 11:00 PM - 7 :00 Am Kenya Time


(This aligns with late evening to early morning hours in Kenya)

About the Role

We are seeking a reliable and compassionate Patient Care Coordinator to support after‑hours operations for a healthcare/home care organization. This role ensures uninterrupted patient care by managing incoming calls, handling urgent needs, and coordinating last‑minute schedule adjustments during evening and overnight hours.

The ideal candidate is calm under pressure, highly organized, and capable of working independently in a fast‑paced remote environment.

Required Skills & Qualifications
  • Excellent communication and problem‑solving skills.
  • Strong organizational skills with the ability to multitask.
  • Comfortable working independently in a remote setting.
  • Professional, calm, and reliable when handling urgent situations.
  • Stable internet connection and ability to work evening/night hours consistently.
Preferred (Not Mandatory)
  • Experience in home care, healthcare, or a related field.
  • Familiarity with caregiver coordination or schedule adjustments.
  • Experience with documentation and compliance tracking.
  • Ability to make quick decisions under pressure.


Responsibilities
Key ResponsibilitiesCall Management & After‑Hours Support
  • Answer incoming calls from clients, caregivers, and referral sources.
  • Respond to after‑hours messages routed through the answering service.
  • Triage concerns and escalate urgent matters when appropriate.
  • Maintain clear, professional, and timely communication.
Schedule Adjustments (Not Full Scheduling)
  • Manage last‑minute caregiver call‑outs and shift changes.
  • Adjust existing schedules to maintain uninterrupted coverage.
  • Notify caregivers and clients promptly of schedule changes.
  • Support continuity of care during evening and overnight hours.
Administrative Support
  • Update caregiver records, including certifications, compliance items, and expiration dates.
  • Track training records and required documentation.
  • Assist with onboarding and general administrative support as needed.
  • Ensure all systems, records, and logs are accurate and up to date.
Caregiver & Client Coordination
  • Serve as the main point of contact for caregivers working night shifts.
  • Provide real‑time problem‑solving and operational support.
  • Promote a positive experience for both caregivers and clients during after‑hours operations.
